(Overview)Nobody sets out to keep a medical record for sixty years. This one has to last that long without anyone tending to it.
(The brief)A first injection at birth and a booster decades later go into the same record, on phones nobody has bought yet. And the record is only worth keeping if clinics use it, so the site had to explain itself to patients and sell a listing to practitioners without either one feeling like an afterthought on the other's page.

Website

(The split)A parent, an adult and a traveller want three different things from the same record. Splitting the site three ways cost less than compromising.
(What shipped)The home page leads with one line: your free digital vaccine record for life. Practitioners get their own route in from the same page, and the questions are sorted by category rather than stacked in one long list a worried parent has to scroll.

A record is only lifelong if it outlives the phone it started on.
